CCS is not a health insurance program. CCS can authorize and pay for specific medical services and equipment provided by CCS-approved specialists with prior authorization. In addition, CCS offers a medical therapy program that provides occupational and physical therapy services to medically eligible children. CCS will not meet all of your child’s health needs, only those related to the CCS eligible condition. | Program Requirement Eligible medical conditions covered by CCS include conditions that are physically disabling or require medical, surgical, or rehabilitative services. There may also be specific other criteria that help determine if a medical condition is eligible. | Financial: | - Family income must be less than $40,000 or
- Child's out of pocket medical costs are expected to exceed 20% of family's adjusted gross income or
- Child has Medi-Cal or insurance that has appropriately denied a medical treatment.

Client Application Process A CCS referral form can be completed by anyone. However, a CCS application for services must be completed by the child’s parent or legal guardians.
