        A data driven plan with built in accountability including realistic, impactful strategies to advance health equity and improve the health of our community.

        Explains how Solano County Health and Social Services/Public Health, partners, and stakeholders will collaboratively address priority health issues over the next three to five years.

        Based off the identified needs in the Community Health Assessment (CHA).
