The Solano County Community Health Improvement Plan is a collaborative process of identifying health opportunities and strengths as a community and improving the health of our county.

What is a Community Health Improvement Plan (CHIP)?

•        A data driven plan with built in accountability including realistic, impactful strategies to advance health equity and improve the health of our community.

•        Explains how Solano County Health and Social Services/Public Health, partners, and stakeholders will collaboratively address priority health issues over the next three to five years.

•        Based off the identified needs in the Community Health Assessment (CHA).
